There are two places to find form responses or submissions from clients. They are be found inside a client's profile or where you create a form from the program's icon.
From a client's profile select forms then you will be able to view all of the submissions for the client's profile you are viewing.
Client's Profile:
When you go into Programs select forms then you will want to click on the number under responses.
Programs > Forms > Responses
When you select the responses from a specific form, you will see the form title at the top of the page next to the x.
You will then be able to view which client left the response.
